Clinical Burden of Post-Ablation Atrial Fibrillation Recurrence in East Asia


Recently, the collaborative research project between Normin Health and Xiangya Hospital of Central South University, titled "Clinical Burden of Post-ablation Atrial Fibrillation Recurrence in East Asia: A Systematic Literature Review," was successfully published in the SCI journal Arrhythmia & Electrophysiology Review. Adhering to PRISMA guidelines, the study conducted a comprehensive systematic review and meta-analysis to evaluate clinical characteristics, recurrence patterns, subsequent treatments, and long-term clinical outcomes among patients experiencing atrial fibrillation (AF) recurrence after catheter ablation in East Asia.
Key Highlights of the Study:
Study Cohort & Evidence Base: The study synthesized evidence from 68 observational studies published between 2019 and 2024 across international and Chinese databases, encompassing a pooled cohort of 28,750 East Asian patients.
Major Findings:
High Baseline Cardiovascular Burden: The mean age of recurrent AF patients was 62.3 years (67.5% male; 53.4% paroxysmal AF). High prevalence rates of comorbidities—including hypertension, hyperlipidemia, diabetes, and heart failure—highlight a substantial underlying cardiovascular risk profile.
Disparities Across Recurrence Patterns: Patients with persistent AF recurrence were older, exhibited wider left atrial diameters, and carried more cardiovascular risk factors than paroxysmal cases. Late recurrence was linked to pronounced cardiac structural remodeling and increased risks of coronary artery disease and bleeding events.
Residual Risk Following Reintervention: Notable recurrence rates were observed after repeat ablation (21.4%) and antiarrhythmic drug therapy (19.6%), underscoring persistent management challenges in clinical practice.
Critical Evidence Gaps Identified: Significant evidence gaps remain regarding long-term hard endpoints (e.g., stroke, major bleeding, cardiovascular mortality) and health-related quality of life, underscoring the urgent need for long-term real-world prospective registries.
Clinical and Policy Implications:
This large-scale synthesis of nearly 30,000 East Asian patients establishes crucial prognostic benchmarks for post-ablation clinical management and risk stratification. The findings also provide valuable evidence to guide future clinical trial design, real-world evidence (RWE) development, and value assessment for innovative electrophysiology technologies.
