Real-World Performance of EQ-5D-5L in Chinese Patients with Hematologic Malignancies

Recently, the collaborative study between Normin Health and the hematology team at Xiangya Hospital, Central South University (led by Prof. Yajing Xu), titled "Reliability and validity of EuroQol-5 Dimensions-5 Levels in patients with haematologic malignancies: a cross-sectional study in China," was successfully published in the SCI journal BMJ Open. The study evaluated the reliability, validity, and concordance of the EQ-5D-5L instrument against health preferences elicited via Time Trade-Off (TTO) across Chinese patients with multiple myeloma, acute leukemia, and lymphoma.

Study Design & Measurement Framework: A cross-sectional investigation enrolling 158 hospitalized hematology patients, systematically comparing EQ-5D-5L psychometric metrics against direct TTO utility measurements across distinct disease subtypes.


Major Findings:

  • Strong Performance in Multiple Myeloma: The EQ-5D-5L exhibited superior internal consistency (Cronbach's α = 0.899) and strong correlation with TTO scores in multiple myeloma patients.

  • Subtype-Specific Measurement Nuances: The instrument showed moderate performance in acute leukemia and was less sensitive in capturing subtle health-state shifts in lymphoma, indicating that generic instruments perform unevenly across hematological cancers.

  • Utility Discrepancies and Covariates: Generic EQ-5D utility scores were systematically higher than direct TTO-derived values across all disease groups, and multivariate regression identified specific clinical comorbidities that impact utility measurement.


Clinical and Policy Implications:

This study cautions against a uniform approach to health utility measurement across hematologic oncology. The findings provide empirical guidance for health economists and trial designers when selecting utility weights, incorporating disease-specific PRO instruments, and modeling QALYs for innovative oncology therapies.
